This proposed project aims to implement and evaluate Health Styles in the Enhanced Rehabilitation Programme for major trauma survivors in GM. Objectives: Through the collaboration of MFT, MIHP and MMU, implement the population segmentation, using the Health Styles questionnaire, into the ERP to tailor care. Evaluate the implementation of population segmentation using qualitative approaches with clinicians and patients. Evaluate the use and relationships of Health Styles to outcomes using audit. Mapping patient Health Styles to available health and social care in GM to tailor care effectively. Building towards a large feasibility study and then RCT to implement tailored care using Health styles in GM. It is a 12 month project starting within 6 months of Feb [the decision date]. There will be 5 focus groups.
